Form 1	Declaration by the institution purchasing high speed diesel oil for resale to members who are fisherman or to fisherman who are members of any other fisherman’s co-operative society in operation in the State of Gujarat.
Form 2	Certificate by a dealer purchasing high speed diesel oil for use in the manufacture of goods taxable under the Gujarat Sales Tax Act, 1969 (Guj. 1 of 1970)
Form 3	Certificate by Messers Vaydoot purchasing aviation turbine fuel for use by it in the aircrafts operated in the State of Gujarat.
